Information from American Cancer Society

CPS-3 is open to individuals who are willing to make a long-term commitment to the study, are age 30-65, and have never been diagnosed with cancer (excluding basal cell or squamous cell skin cancers).

Here is what you need to do to enroll:

1) Visit http://cancerstudytx.org to choose an enrollment time, date, and location. Simply click on Lubbock and select one of the 4 locations. From there you will be able to schedule a specific time on the date you have chosen. You may also call 888-604-5888 to schedule a time.
2) Once you have registered, you will be emailed a link to the comprehensive health survey. This will take 30-45 minutes to complete, but you do have the ability to save your progress and return later.
3) At the conclusion of your survey you will receive an Enrollment Confirmation page. Please print this page if possible and bring it with you to your September enrollment site.

At enrollment you will spend approximately 25-30 minutes to complete a brief enrollment form, have your waist measured and give a small blood sample. Once you are enrolled, you will receive follow up surveys every 2-3 years to complete in your home.

That's it! In the time it takes to eat dinner, you can make a difference in the fight against cancer!

But it doesn't stop there -- won't you commit to telling a friend, relative, or coworker about your participation? You surely know someone who has been affected with cancer -- share the website with them and give them the opportunity to fight back in their own lives!  Their participation will help us better understand what causes or prevents cancer and will ultimately save lives from cancer for this and future generations. Here is a 2 minute video about the study for you to enjoy:  http://www.youtube.com/watch?v=hWBYpVWG-4Y

If you are willing to volunteer at one of enrollment locations, please let us know by calling or emailing.
Thank you so much for your participation. If you have any questions or problems, please don't hesitate to let us know. Together, we can make a world with less cancer and more birthdays!
